1 The mechanism of injury is typically a fall with the wrist extended and in ulnar deviation. Scapholunate dissociation (SLD) is the most common type of carpal instability. It is the most common instability pattern because the intercarpal motion during flexion and extension cross at the scapholunate interface. Another reason is that compression forces transmitted through the capitate has a tendency to separate the scaphoid and lunate.

A grip compression view or wrist in ulnar deviation may be necessary in addition to PA/lateral views.

In a nutshell, scapho-lunate dissociation is when these two bones are forced apart due to trauma. Normally they sit closely beside each other and are held in place by ligaments. In this injury, the two bones are spread apart and the ligament that holds them close together is torn.

A person who experiences pain in the back of the wrist or the thumb side of the wrist (following a fall) should be examined for scapho-lunate dissociation. A grip compression view or wrist in ulnar deviation may be necessary in addition to PA/lateral views.
