This pattern of ulnar translocation, in reality, represents a severe form of scapholunate dissociation and carries a worse prognosis. 19 Figure 19: T1-weighted coronal image demonstrates ulnar translocation of only the lunate (L) and triquetrum (T) bones [radsource.us]
1 The mechanism of injury is typically a fall with the wrist extended and in ulnar deviation. Scapholunate dissociation (SLD) is the most common type of carpal instability. It is the most common instability pattern because the intercarpal motion during flexion and extension cross at the scapholunate interface. Another reason is that compression forces transmitted through the capitate has a tendency to separate the scaphoid and lunate.

It is the most common instability pattern because the intercarpal motion during flexion and extension cross at the scapholunate interface. Another reason is that compression forces transmitted through the capitate has a tendency to separate the scaphoid and lunate. Scapholunate dissociation results from an injury to the scapholunate interosseous ligament which joins the scaphoid and lunate wrist bones together. This ligament is often torn by a fall onto an outstretched hand. This can tear the ligament, causing the wrist bones to move apart or ‘dissociate’ from one another when the wrist is used. A grip compression view or wrist in ulnar deviation may be necessary in addition to PA/lateral views.
In a nutshell, scapho-lunate dissociation is when these two bones are forced apart due to trauma. Normally they sit closely beside each other and are held in place by ligaments. In this injury, the two bones are spread apart and the ligament that holds them close together is torn.
A person who experiences pain in the back of the wrist or the thumb side of the wrist (following a fall) should be examined for scapho-lunate dissociation. A grip compression view or wrist in ulnar deviation may be necessary in addition to PA/lateral views.
Definition. Ruptur av det interosseösa ligamentet mellan scaphoideum och lunatum (SL-ligamentet), vilket medför att scaphoideum tippar volart och lunatum dorsalt.

Scapholunate dissociation is when the small bones in the wrist (the scaphoid and the lunate) move excessively in relation to one another. Physiotherapy is an important part of scapholunate dissociation rehabilitation.

c/o severe pain in the right wrist. Frontal view of the wrist showed increase in scapholunate distance (arrow). It measures 3.5mm. Other intercarpal joints measures about 1.5mm only. This is consistent with Terry-Thomas sign and indicates scapholunate dissociation.

Xray indicates scapholunate ligament instability when the scapholunate distance is more than 3 mm, which is called scapholunate dissociation. A static instability is generally readily visible, but a dynamic scapholunate instability can only be seen radiographically in certain wrist positions or under certain loading conditions, such as when clenching the wrist, or loading the wrist in ulnar deviation. 2019-04-02
Scapholunate dissociation is a rotatory subluxation of the scaphoid with widening of the gap between the scaphoid and lunate.

Scapholunate dissociation rapidly is becoming the most common form of carpal instability. Despite the literature supporting the importance of early diagnosis and treatment, this problem is not often recognized on initial presentation.

Scapholunate dissociation is the most common and most significant ligamentous injury of the wrist. [1] [2] Scapholunate instability is the most frequent pattern of carpal instability occurring separately and as part of other wrist disorders. [3]
